Starting the journey to recovery is a courageous step for anyone struggling with a mental health disorder or addiction. At Resolve, we understand that entering a psychiatric rehabilitation program (PRP) is a life-changing decision. That is why our Program Director and Mental Health Associates (MHAs) strive to make your path to recovery a rewarding experience of mental, physical, and personal growth.
Qualifications for PRP Services
Resolve provides PRP services to adults diagnosed with a mental health disorder, persons struggling to overcome an addiction, or individuals challenged by anger management issues.
Individuals seeking treatment must be:
- 18 years or older;
- Diagnosed with a disorder or illness approved for PRP services; and
- Eligible for coverage under Medicaid or private insurance.
Self-Admitting and Referral Clients must complete the following forms:
- Authorization for the Release of Information Form
- HIPAA Acknowledgment Form
- Agreement to Treatment Form
- Psychiatric Rehabilitation Referral Form (To be completed by a licensed clinician.)
